COVID-19 Waiver  

ACKNOWLEDGEMENT OF RISK AND WAIVER OF LIABILITY
RELATING TO ATTENDING EVENTS AND COVID-19

I have voluntarily opted to participate in the 2022 Women in White Coats Conference and Wellness Retreat in-person event (the “Event”). I understand and acknowledge that, when I participate in the Event, I may interact with the instructors, other participants, and/or members of the public. Any one of them may be carriers of COVID-19. Women in White Coats Project LLC (“WIWC”) cannot guarantee that the instructors, other participants, or other members of the public are not carriers of COVID-19.

I understand and acknowledge that, by participating in the Event, I may be exposed to COVID-19 and that there is a risk that I may become infected with COVID-19 if I participate in the Event. Although WIWC has taken reasonable precautions to mitigate the risks associated with COVID-19, consistent with the Guidance published by the Centers For Disease Control and Prevention (the “Guidance”), and is providing space that is free from recognized hazards that are causing or are likely to cause death or serious physical harm to participants, WIWC cannot prevent me and/or my other contacts from becoming exposed to, contracting, or spreading COVID-19 while participating in the Event.

I agree to present proof of COVID vaccination or a negative COVID Test, taken no more than 72 hours prior to the start of the event, upon in-person Event registration. I understand that I will not be permitted to participate in the event unless I provide this documentation.

Covid-19 Exposure Representation:

By accepting this Waiver, I represent that I will only attend the Event if:

• I (and everyone in my household) are not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell;
• I attest that I am fully vaccinated (including booster) or will show proof of a negative PCR COVID testing no less than 72 hours before the start of the event.

Acknowledgement of Covid-19 Risk at Event:

By accepting this Waiver, I expressly acknowledge that I am aware of and, to the greatest extent permissible by law, accept all the risks associated with participating in the Event, during the COVID-19 pandemic. My signature certifies that I have been advised of and am aware of the risks involved, and the decision to participate in the Event is voluntary and of my own volition.

Assumption of Risk:

I have read and understood the above warnings concerning COVID-19. I understand some risks associated with COVID-19 are unknowable in advance. I hereby choose, to the greatest extent permitted by law, to accept the risk of contracting COVID-19 in order to participate in the Event.

Waiver of Liability:

I, on behalf of myself and my heirs, successors and assigns, hereby forever release, waive, discharge, hold harmless, defend, and indemnify, to the greatest extent permitted by law, WIWC, and its CoFounders, parents, subsidiaries, related entities, affiliates, predecessors, successors, assigns, insurers, owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives (“Releasees”), of and from any and all claims, known and unknown, asserted or unasserted, which I have or may have against Releasees in connection with exposure, infection, contact, transmission and/or spread of COVID-19 related to my participation in the Event.

I understand that this Waiver means I give up my right, to the greatest extent permitted by law, to bring any such claims against the Releasees, including for any COVID-19 related personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence, and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

By accepting this notice, I (1) acknowledge that I have been advised of the risks regarding COVID-19 while participating in the Event, (2) assume all risks of exposure to COVID-19 while participating in the Event, and (3) knowingly and voluntarily waive, release, and agree to hold the Releasees harmless from any COVID-19 related harm resulting from your participation in the Event.

Choice of Law:

You understand and agree that the laws of the State of Delaware will apply to this Waiver.

By accepting this waiver, I agree that I have carefully read and fully understand all provisions of this waiver, and freely and knowingly assume the risk and waive my rights concerning liability as described above.

Refund Policy

Full refunds will be given through January 31, 2022 with a written request (to info@womeninwhitecoats.com) received by that date. After January 31, 2022 there will be NO REFUNDS given for any reason including illness (COVID-19 or other) or due to injury or other inability to travel.

Tickets are transferable and must be arranged by the participant by April 15, 2022. WIWC will not arrange a transfer to another participant.

In the event that the entire in-person conference is cancelled due to state or federal regulations or other natural disaster beyond our control NO REFUNDS will be given. Your ticket will be transferred to our 2022 virtual conference and you will be given a voucher to use for our next in-person event or other programs in the amount of the difference between your ticket price minus the initial ticket price for the 2022 virtual event.

Media Release Consent Form

Women in White Coats Project LLC (WIWC) creates marketing campaigns on an ongoing basis, showcasing awards, testimonials, and/or examples of coaching or teaching within its Programs, Coaching Programs, and/or Doctors Lounge and Events (the “Marketing Campaigns”). The Marketing Campaigns may include advertisements on social media, in emails, on Women in White Coats websites, in print materials, or other online materials (collectively, the “Advertising”). The Advertising may feature still images, recordings, videos, quotes, testimonials, or recordings of coaching that take place on live coaching calls and/or at in-person events.

This consent form will authorize Women in White Coats LLC to use and print photographs, and/or to use testimonials, videos, recordings, and any other media material containing my name, likeness, voice, and/or image that I provide or which is captured during a live coaching call or at an in-person event (the “Materials”), for educational, informational, and promotional purposes, including in its Advertising.

The Materials may be used in all, but are not limited to, the following: Women in White Coats Project LLC Advertising, newsletters, emails, newspaper or other print articles, web listings, websites, etc.

I also voluntarily release WIWC and any affiliated or related persons or entities from any and all claims and causes of action based upon WIWC use of the Materials, and grant WIWC permission to use, publish, exhibit, and/or reproduce the Materials in any manner and in any and all media now known or later developed, without obligation to pay me any royalty or compensation of any kind. Such rights include but are not limited to use in connection with advertising, social media, publicity, marketing, fundraising, promotional, and other commercial purposes designed to promote the School in a positive manner.

I have carefully read this release and understand it and am authorized to and do agree to its terms. I understand that my acknowledgement and acceptance will be kept on file by Women in White Coats.
